How do I restore it?

Check Settings > Genreal > Restrictions and make sure you have not turned off the camera.

Hi Herb. - welcome to the forum!

If TP's advice does not solve your issue, then make sure you have not moved the 'camera icon' to another home screen or into a folder; if that still is not the solution, then 'Reset' the iPad and see if the icon reappears (if not familiar w/ the process, hold down both the Start & Home buttons simultaneously until the Apple logo appears, release, and let the device reboot - none of your apps, data, or settings will be affected) - Dave
