Cannot reinstall faulty purchased app

I downloaded and purchased an app and it does not work. I have received my receipt from iTunes for the purchase.
This app is not listed under purchased on my iPad so I cannot reinstall it.
Any suggestions anyone?
 
This may be because it is no longer in the app store. Not sure if there's anything you can do. Sorry. :(
