Fill-in forms on i-pad

Can anyone recommend an application that will allow me to create a fill-in form on my i-pad and be abke to fill it in as needed?

I've used FormEntry for several customers and it works very well.

There are a bunch of options though, so keep looking and make sure you really understand what you want to do - cause you can probably do it
