Form on IPAD

Hi,

I would like to create a form, then, when you click on "Send" : the variables are used in the generated files, which I would to send by mail.

Explanation :

I have a form (NAME : John)

Click on "Send"

File is completed with the variables (Hello, my name is John)

Then I want to send it by mail with this attached file

How is it possible ?
