Log book format

I'm looking for a log book application with a date field and unlimited (?) field for note taking.... Something that is a little more formatted. Is a spreadsheet my best option ?

I use "Notes Plus" for something similar. I have my notes and such separated in different folders and you can name them as anything (including the date) so they are easy to find.

Not entirely sure what you are after but I use smart pad app. It allows me to record notes (I use it for a diary at work) so the notes are linked to the calendar. Might be worth checking out to see if it will do what you want or I can test it out for you if you have specific requirements.
