I hope this is the wright place for this question.
I am looking for a app that can scan a form convert it into a data entry form and when compete be able to print the form in the same format as the original form but to save the patients details for future reports. similar to a electronic patient record but not as complex.
May be a app builder could help I would be happy to reward them for this. If I am allowed to say that on this site.
