App's able to load files and be able to write on them??

Im trying to find out if theres a app that will allow u to load a file and be able to write on it..

Ex) checklist or evalutation report!

Take off time: _______ am or pm.

a app that would let me fill in the blanks from a evalutation needed on a daily basis.

There's an app called SignMyPad that a number of Forum members are using. It allows you to 'sign' and fill in boxes on a PDF document. So you could create a template document in, say, Apple 'Pages', save it as a PDF and then use SignMyPad to annotate/sign/fill-in the blanks.
