Free Survey Creator?

Hi, I am trying to be resourceful and save the Earth by not wasting a tons of paper on creating a bunch of surveys.
I'm looking for a free survey application for the iPad, that will allow me to use open-ended questions, numeric questions, and categorize questions.

If anyone know of any free survey creating apps, please let me know!
