ipad trouble ticket software

im looking for a program where people can bring in there items for me to fix i can put it in my ipad the date and there name and what im fixing then i need a place to make notes for what i did and have a spot for status so i can let the person who brought it in what is going on with there item im fixing so what program could i get that can do that thanks?

i also need a program that works on both the pc and ipad or a website so i can use it from different locations some with out ipads
