What you need

Scissors
Credit card sized sim card with a micro sim card that you can pop out - vodafone currently send these out for free, o2 are free but hit and miss whether you get a small sim or micro sim that can pop out
Small sim card

Cut out a small sim card from the large sim card using the small sim card as a template

Excuse my ignorance and curiosity, but why would you need to do this?

If you have a SIM that you want to swap between an iPad and a regular smart phone. The iPad takes a microSIM and most smart phones (iPhone excepted) take the more 'normal' and larger sized SIM. The adaptor holds the microSIM so that it can fit into a normal SIM card holder.
