Consent form app?

Hi all, new to the forum.
Was searching for an app and it brought me here.
I found an app called promo form which was almost perfect, but not at the subscription price.

I want to have a consent form for clients to read and sign.
Needs be boxes to print the name, then sign.
Ideally it will then store on the ipad to upload or will email direct to an account.
It needs to be idiot proof / very user friendly, so people can just click the boxes, then a big end or save button.

Might not be quite as idiot proof as you'd like, but FormsConnect will do everything you listed.

I did read somewhere else about forms connect, but couldn't find it in the App Store.
