Ok, eyes have adjusted. The top is a screw on cap with a tamper lock.

I'm guessing it's a plastic juice/drink bottle. The kind you usually get for the kids. No idea what brand.

It is a cap for a plastic bottle, Coca Cola, to be precise.

The Admiral was close, but yours is the answer I was looking for. Though in this case, the thing twisted into the corkscrew is a jewelers screwdriver; very handy for repairing eye glasses.
